Full Body Massage: What Actually Happens and What to Expect

A comprehensive guide to what a full body massage involves — the sequence, the techniques, what you'll experience, and how to get the most from the session.

Full body massage therapy guide

"Full body massage" is one of those phrases that sounds clear but leaves most people with unanswered questions: What does "full body" actually mean? Do they work everywhere? What's the sequence? Do I need to undress completely? What should I expect to feel?

This guide answers all of it.

What "Full Body" Actually Covers

A professional full body massage covers:

  • Back: The whole of the posterior torso — lower back, mid back, upper back, the spaces between the shoulder blades
  • Shoulders and neck: The upper trapezius, levator scapulae, and neck muscles (typically worked face-down and sometimes repeated face-up)
  • Glutes and hips: The gluteal muscles and hip rotators — areas that are worked in good full body massages and frequently skipped in less thorough ones. Crucial for lower back and hip health
  • Legs (back and front): Hamstrings, calves, and feet face-down; quadriceps, IT band, tibialis anterior, and feet face-up
  • Arms and hands: Biceps, triceps, forearms, and hands
  • Head, neck, and sometimes face: Scalp massage, sub-occipital release, and sometimes a brief facial massage — some therapists include this as standard, others offer it as an optional addition

The sequence matters. Most therapists work face-down (prone) first, covering the back, shoulders, glutes, and legs. You'll then be asked to turn over (supine) for the front of the legs, arms, chest, and head.

What the Draping Looks Like

A professional massage maintains your privacy throughout the session through draping — a sheet or towel is used to cover the areas not currently being worked on. Only the specific body part being actively massaged is exposed at any time.

This is managed by a skilled therapist without interrupting the flow of the treatment. When you turn from prone to supine, the therapist will hold the sheet to maintain coverage as you move.

Your comfort is paramount. You can wear underwear throughout if you prefer — it limits access to some areas slightly but is entirely your choice. Some people are more comfortable in minimal clothing; others prefer to keep underwear on. Both are fine.

The Experience Across 90-120 Minutes

The first phase (face-down) typically occupies 40-60 minutes for a 90-minute session. This is often the most therapeutic part — the back, shoulders, and glutes are where most people's significant tension lives.

Lower back: The session often begins here, with warming effleurage that gradually deepens into the paraspinal muscles. The quadratus lumborum (deep lower back muscle, a common source of lower back pain and hip issues) is typically addressed here.

Upper back and shoulders: The area most people want the most attention. Deep effleurage between the shoulder blades, work on the rhomboids and middle trapezius, and specific work on the upper trapezius and base of neck.

Neck (face-down): Sub-occipital release — work on the small muscles at the base of the skull that are chronically compressed in forward head posture. One of the most consistently relief-producing moments in a full body treatment for most clients.

Glutes: Worked with the client face-down, the gluteal muscles (which often contribute significantly to lower back and hip pain) receive deep work.

Back of legs: The hamstrings, calves, and feet.

The second phase (face-up) typically occupies 30-45 minutes for a 90-minute session:

Front of legs: Quadriceps, IT band, shins, feet (from the front).

Arms and hands: Upper arms, forearms, hands. Hand massage in particular is deeply relaxing for people who type a lot.

Chest and anterior neck (where appropriate): Light work on the pectorals and front of the neck is available if wanted — some clients find this deeply releasing; others prefer not to have this area worked.

Head and face (if included): Scalp massage and sub-occipital release from the front. Some therapists also include a brief facial massage.

Getting the Most from a Full Body Session

Communicate before: Tell your therapist about any areas you want more focus on, any you want avoided, any conditions or injuries. This shapes the session significantly.

Communicate during: Too much pressure? Not enough? A particular spot that needs more attention? Therapists welcome feedback — it allows them to give you a significantly better treatment.

Receive passively: Try not to "help" — don't lift your arm to assist when the therapist moves it, don't tense in anticipation. The work is done to you; your job is to let it happen.

Allow time afterwards: Don't book a full body massage immediately before you need to be somewhere. The post-massage state is disorienting in the best possible way — you need at least 30 minutes of calm afterwards, and ideally the rest of the evening.

Frequently Asked Questions

What does a full body massage include?

A professional full body massage covers the back and spine, shoulders and neck, arms and hands, glutes and hips, legs (front and back), and feet. Some therapists include the scalp and face; others offer this as an optional addition. The treatment is typically performed in sections, with you repositioning from face-down to face-up at a midpoint.

How long should a full body massage be?

90 minutes is the minimum for a truly comprehensive full body massage. 120 minutes allows for more thorough attention in each area and a less hurried experience. 60 minutes can only cover a full body in a fairly superficial way — better suited to a focused partial treatment.

Do I need to undress completely for a full body massage?

Most professional full body massage is performed with the client undressed to underwear (or completely), lying under a sheet or towel. Only the area being actively worked on is exposed at any time — your privacy is maintained throughout by careful draping. Your comfort level and choices are entirely respected.
